Bonus – Data-Driven Footwear Prescription: Structuring Patient Factors for Real-World Application in Diabetic Foot Care

September 18 @ 14:35
14:35 — 15:30

This session is open to everyone. Participate in this 3-part project over the next few days and gain extra CPD points.

Background:
This study aimed to create a standardized list of attributes for prescribing offloading footwear for individuals with diabetes-related foot ulcers (DFUs). Current guidelines lack structured variables to support systematic data collection and decision-making, despite the importance of patient-specific factors such as clinical condition, activity level, and lifestyle.

Methods:
A scoping review of 81 studies (2014–2025; 5,001 participants) identified both clinical and adherence-related factors influencing prescription. Ninety unique decision factors were categorized using the WHO’s five Dimensions of Adherence. Of these, 48 were input variables and 42 output variables. Only 16 aligned with HL7 FHIR standards; 74 required custom extensions, reflecting a lack of standardization.

Conclusions:
The resulting framework offers a foundation for standardized, interoperable, AI-ready datasets in DFU footwear prescription. Expert review and real-world validation are now needed to ensure clinical relevance, feasibility, and effective application in practice.
